Policy Waiver for Nail and Permanent Makeup Services


In order to provide quality services to all clients, the following policies have been outlined for the benefit of you, and your nail technician.

The policies outlined in this waiver result in coordination of involved parties.

Please read each section carefully  sign as instructed at the bottom of this policy waiver.

Please note all policies are effective immediately upon completion, and are subject to change anytime. By signing you agree to all terms and conditions as outlined in this waiver and accept responsibility to abide by current and future policies.

Providing your email below will allow for communication of your confirmation email and appointment information.

Your email address and all personal information will be kept confidential and will not be shared with any entity for any reason.

Liability Policy

All clients must be in good physical health, including the absence of any skin conditions. It is the client’s responsibility to have consulted a physician who approves the client’s use of any service, products, or techniques. Please note Emilee/Ems Embellishments reserves the right to refuse service if it is determined it could be detrimental to either your health or Emilee/Ems Embellishments health. The client agrees that before he/she uses or obtain any services in the event you determine there is any issue he/she agrees to notify staff immediately of any unsafe conditions.

Cancellation Policy

All appointments are required to be cancelled no later than 24 hours prior to appointment time. Any cancellation or rescheduled appointment after 24 hours is subject to a fee of 100% of schedule service charge.  If you have booked your appointment with less than 24 hours between your appointment and booking time, you are required to cancel your appointment no later than 2 hours prior to your start time to avoid cancellation fee’s.


No Call/No Show Policy

Please note No Call/No Shows are not tolerated.  Should you No Call/No Show you will be charged a fee of 100% of your service. You will not be permitted to reschedule until this fee is paid. Your second No Call/No Show you will be charged 100% of your service fee and you will be required to prepay for your next appointment. Should a No call/ No show occur a third time you will not be permitted to reschedule with Em’s Embellishments for a minimum of 6 months.*** This is discretionary  and Ems Embellishments / Emilee Clark will determine if charge is valid based upon the circumstances.

Refund Policy

We have a no refund policy on all services completed with Em’s Embellishments. We offer free fixes within 3 days of the service completion pending repair is due to a mistake made by Em’s Embellishments.

Late Policy

Please be advised your tardiness impacts all following appointments and clients. Arriving 5 minutes or more past your appointment time is grounds for rescheduling. Please note your service will be altered to fit the remaining time in your scheduled service. If this occurs you will still be responsible for 100 % of your service fee.

Sick Policy

If you are sick or have any symptoms  of an illness, please notify your nail technician immediately and reschedule your appointment. Should you arrive to your appointment and appear physically ill, or are still recovering from an illness your appointment will be cancelled and you will need to reschedule after all symptoms have dissipated.

Babies and Small Children

For the safety of you and your child please do not bring small children or babies to your appointments. Children are not allowed to wait in nail room during appointments. They may remain in the main lobby with an adult of 18 years of age or older. Failure to provide proper supervision will automatically result in rescheduling your appointment. NO EXCEPTIONS.


Returned Payments

All returned payments are subject to a $25.00 fee in addition to service fee. You will not be permitted to reschedule until this fee is paid. Any returned check payment is subject to a $25.00 fee and  penalty  by law if found to intentionally defraud Ems Embellishments. 

Payment Policy

All clients are required to add a card on file at the time of booking. You will not be charged for your service until the time of your appointment.   Payment is due in full at time of service.  Additionally, cards on file will be charged per the policies laid out in this waiver for single incidents and/or repeat violations up to and including deposit requirements prior to scheduling an appointment.

Please also be advised a convenience fee of 3% will be charged for any card that is used as a payment method. Please note other methods of payment free of charge include, Venmo, Apple Pay, Cash App, Cash. Adding your card on file means you understand and accept all policies as outlined in this waiver and authorizes penalty/processing fees to be charged for missed/late/no show appointments.

Refusal of Service

Please be advised repeat violations of these policies and/or refusal to pay penalty fees may result in refusal of service.  Additionally, no services will be rendered until delinquency of returned payments is addressed.



All policies outlined in this waiver are agreed upon by parties associated with services being rendered by Emilee Clark for Em’s Embellishments  inside Beau Monde Spa. These policies are established for Emilee Clark / Em’s Embellishments and are not valid for any other party.  By signing below you acknowledge that all policies previously mentioned within this waiver are binding and are subject to change at any time. Your signature below expresses your agreement to these policies and all future changes or additions. Once ackowledgment is submitted you will automatically be redirected to book your appointment.
